Entering the "Sexy Girls Next Door Amateur Nude Calendar Competition" is easy! First, you MUST read, understand, and agree to, the Official Competition Rules & Regulations which are posted HERE. Then complete the Entry Form below. Fields noted with a "*" are required, but your personal information (i.e. home address, phone number(s), and email address) will NOT be posted online. Only your name, city & state, age, measurements, hair color, eye color, etc. will be included in your online profile. The additional information is for Entry and record keeping purposes ONLY and will not be included online. NOTICE: The prize package for Finalists includes a 7-night stay at Hedonism II from October 18th, 2010, through October 25th, 2010. Hedonism II is "all inclusive" which means all meals and beverages (yes, alcohol too!) are included. Cash value of accommodations is approximately $1,000.00. Air fare is NOT included; each Finalist is responsible for getting herself to Jamaica. All travel arrangements will be made through Hayden Travel & Cruises.

I understand that the photos I am submitting with my Entry Form will be posted on the "Sexy Girls Next Door Calendar" web site for viewing and rating by visitors to this web site, and I am giving my expressed permission for them to be included with my profile. I further agree that if I am chosen as one of the twelve (12) Finalists in this Competition, I am required to travel to Hedonism II, Negril, Jamaica, during the dates of October 18th, 2010 and October 25th, 2010, and compete in the Finals on Saturday, October 23rd, 2010. I understand that as a Finalist, my Prize Package includes accommodations at Hedonism II for myself ONLY; if I choose to take a traveling companion (husband, boyfriend, friend, etc.), he/she is responsible for the cost of their accommodations. I understand that airfare is NOT included in the Prize Package. I/we are responsible for the cost of airfare to and from Jamaica. I/we must book our accomodations and airfare through Hayden Travel & Cruises, co-sponsor of the Competition.

During my stay at Hedonism II, I understand and agree that I will be photographed nude by professional photographer Scott Church, and one or more of those photographs will be featured on the printed 2011 "Sexy Girls Next Door Calendar." I am also giving my permission to the Competition Sponsors to use my photographs for the future advertisement and promotion of the Competition, including but not limited to internet and print media.

I am entering this Competition of my own free will, I have completed this Entry Form myself, and I have not been forced or coerced into doing so by any other party. If I am chosen as a Competition Finalist, I understand that, prior to the Finals, I will be required to provide a notarized signature agreeing to the Competition Terms & Conditions, Rules & Regulations, etc. ***
